If you do not see your plan listed here, please call your insurance plan to confirm if we are in-network.
We must emphasize that as a medical office, our primary and most important relationship is with you, and not your insurance company. We can assist in determining if we are in-network with your plan and to determine your health plan(s) orthopedic benefits and coverage levels, but it is ultimately the patient’s or guarantor’s responsibility as the plan member to know and understand the medical providers and services covered by your health plan(s).
While the filing of insurance claims is a courtesy we extend to our patients, all charges and balances owed for services rendered, are ultimately the patient’s or guarantor’s responsibility. Please inquire and explore your orthopedic benefits with your insurance plan(s) prior to your visits or services rendered. It is also your responsibility to notify us if you insurance plan(s) or policy changes. This will help us prevent any billing surprises or unnecessary visit or surgery cancellations.

Fees & Payments
Copayments, coinsurance, and any other agreed upon fees are due at the time of your visit. This includes surgery deposits, visit charges if you do not have insurance; and payments for any non-covered services such as PRP injection fees, waterproof casting, etc.
For your convenience, we accept most major credit cards, and local checks with ID. We no longer accept Cash payments. No Cash is kept on premise.
